P athophysiology of Parkinson’s disease is well characterized. Dopaminergic neurons in thesubstantia nigra that project to the striatumdegenerates and there is a loss of dopamine in thestriatum (Fig. 1). Many of patients’ motor dysfunc-tions respond dramatically to replacement of thedopamine. Since dopamine itself does not crossthe blood-brain barrier after oral administration,its precursor, levodopa, is given as treatment forParkinson’s disease. Addition of a competitiveinhibitor, carbidopa, to the regimen blocks theconversion of levodopa into dopamine untillevodopa gets inside the brain. This reduces side-effects such as nausea and improves the drug’sefficacy. The combination of levodopa andcarbidopa remains the gold standard of therapyfor Parkinson’s disease to this date.
